A disability is defined as a physical or mental impairment that substantially limits one or more major life activities of such individual.

Students seeking accommodations or services from The 立博体育官网 must submit documentation to verify eligibility under Section 504 of the Rehabilitation Act of 1973 和 the Americans with Disabilities Act (ADA) Amendments Act of 2008.

Eligibility for accommodations is determined on a case-by–case basis upon review of the documentation 和 a collaborative intake interview appointment. The documentation must include a copy of the most recent evaluation, including testing results 和 a written summary. 文档 must provide an assessment of the current limitations of the disability on major life activities 和 recommendations for appropriate accommodations.

Please note that Individualized Education Plan (IEP) 和 504 Plan are helpful, but are typically not considered as sufficient documentation to st和 alone when determining eligibility.

注册 for accessibility services is voluntary 和 it is the student’s responsibility to initiate 和 maintain contact with our office. The student is also responsible for submitting any 请求ed documentation to 接入性能的服务, 与他们的入学申请分开.

• 文档 may be mailed, emailed, faxed, or dropped off at our office. If you need to obtain documentation 和 do not have a current provider, please review the 校外推荐 from Counseling 和 Psychological Services at the 立博体育官网. Please note that the 接入性能的服务 does not endorse the providers on this list 和 it is the student’s responsibility to verify that the providers can supply sufficient documentation.
• Once you have submitted your documentation, we will email you on your 立博体育官网 email account to schedule an Intake Interview appointment. If your documentation is not considered sufficient, you may be granted provisional accommodations for one 学期 while you continue to gather additional required documentation.
• Intake Interview appointments usually last between 45 minutes 和 one hour 和 consist of completing paperwork, discussing the nature 和 limitations of the disability, 和住宿. 在此任命期间, an 接入性能的服务 staff member will determine which accommodations you will be approved for.

• It is the student’s responsibility to speak with their professor to use or waive specifically approved accommodations. It is also the student’s right to use or waive approved accommodations on a case-by-case or 学期-by-学期 basis.

An Accommodation Letter is a 立博体育官网 document issued by 接入性能的服务 to a student who has filed appropriate documentation verifying the presence of a disability 和 who is eligible to receive specific services 和住宿. 在高等教育阶段, accommodations are not automatically provided to students, 和 professors are unable to provide accommodations without receipt of this letter.

It is the student's responsibility to contact 接入性能的服务 at the start of 学期 申请新的住宿通知书. This process must be repeated each 学期 for each professor 和 for each class you wish to receive accommodation support. 住宿信 are sent electronically to faculty 和 the student is copied. Accommodations are not provided retroactively.
